Abstract:

Hydatid disease is a parasitic infection mainly caused by Echinococcus granulosus and is endemic in many parts of the world. Although hydatid disease can be found anywhere in the human body, the liver and lungs are the most commonly involved organs. Urinary tract involvement has been seen in about 2%-4% of the cases; however, the isolated renal CYST is extremely rare. Here, we report a 5-year-old boy with an isolated huge renal hydatid CYST with no scolices or hooklets in aspirated fluid mimicking a SIMPLE renal CYST. The clinicians and radiologists should consider hydatid disease in the differential diagnosis of CYSTic lesions found in any part of the body especially in endemic countries since earlier diagnosis is crucial for appropriate treatment.

PURPOSE: TO EVALUATE ENDOSCOPIC SURGERY OF SIMPLE RENAL CYST ABLATION AND COMPARISON OF RESULTS WITH RESULTS OF SIMPLE RENAL CYST ASPIRATION WITH INJECTION OF SCLEROSING AGENTS IN OTHER CENTERS. METHODS: FROM JUN. OF 2001 TILL JUN. OF 2003 IN A PROSPECTIVE STUDY 10 PATIENTS WERE STUDIED, WHO PRESENTED WITH FLANK PAIN IN UROLOGIC CLINIC AND IN WORKUP REVEALED SIMPLE RENAL CYSTS IN ALL PARTS OF KIDNEY EXCEPT PAR PELVIC AND HAD NO PREVIOUS SURGERY ON SAME SIDE. THESE PT'S UNDERWENT ENDOSCOPIC CYST ABLATION THEN FOLLOW UP WAS DONE 2 WKS, 2 MOS, 6 MOS AND 12 MOS AFTER OPERATION WITH VISIT AND SONOGRAPHY FOR EVALUATION OF FLANK PAIN, POSSIBLE COMPLICATIONS, AND SIZE OF RENAL CYSTS. RESULTS: FROM 10 PT'S UNDER STUDY IN 1 CASE THE ACCESS TO RENAL CYST WASN' T SUCCESSFUL DUE TO SMALL SIZE OF RENAL CYST (34MM), AND IN 9 OTHERS CASE, THE PROCEDURE WAS DONE WITHOUT SIGNIFICANT COMPLICATION. ALL PT'S WERE FEMALE WITH AVERAGE AGE 55 Y/O. MEAN RENAL CYST DECREASED MORE THAN 50% FROM SIZE PREOPERATIVELY SO THE PROCEDURE WAS SUCCESSFUL IN ALL CASES. IN 8 CASES (88%) PAIN IMPROVED COMPLETELY.1 CASE DEVELOPED LEAKAGE FOR 6 DAYS. AVERAGE OF FOLLOW-UP PERIOD WAS 11.4 MOS (6-22 MOS). MEAN OPERATIVE TIME WAS 38 MIN. MEAN HOSPITAL STAY WAS 3 DAYS. CONCLUSIONS: REVIEW OF THE GOOD RESULTS AND COMPARING WITH RESULTS OF OTHER CENTERS WHERE ASPIRATION OF CYST ABLATION, REVEALED THAT; IN SPECIAL CASES THAT LESS INVASIVE PROCEDURES AS ASPIRATION DEVELOPED RECURRENCE OR LAPAROSCOPIC SURGERY CAN NOT POSSIBLE, THIS ENDOSCOPIC SURGERY AN USE AS A GOOD ALTERNATIVE THERAPEUTIC CHOICE.

Abstract:

SIMPLE renal CYSTs are one of the most common lesions in elderly. These CYSTs are usually asymptomatic but when the size of these CYSTs increase, we would see symptoms such as hypertension, hematuria, flank pain or urinary obstruction. In this study, we explore a case of small bowel obstruction that presented with nausea, repeated vomiting that causes hematemesis, and a submucosal obstructive lesion that was seen in Esophagogastroduodenoscopy (EGD). After endoscopic ultrasound (EUS) evaluation, we detected a large SIMPLE renal CYST and approved our diagnosis with CT scan. We planned a medical treatment for this patient that consist consuming small size meals, 5 to 6 times a day, and high calorie liquids in small volumes. We conclude that SIMPLE renal CYST can be one of the cause of extrinsic intestinal obstruction and EUS is affective for differentiation of intrinsic submucosal lesion from extrinsic compression.

Abstract:

Despite their names, SIMPLE bone CYSTs are no longer categorized as CYSTs since they lack an epithelial lining. However, their nature remains controversial. The internal structure is totally radiolucent, sometimes showing multilocular appearance, although the lesion does not contain true septa and the ridges of bone is produced by the scalloping effect. We presented two cases of histopathologically confirmed SIMPLE bone CYST. Radiographic features such as multilocular appearance and significant buccal and lingual expansion are not usual findings for SIMPLE bone CYST, whereas evident in our presented cases.

Abstract:

Background and Objective: Adrenal CYSTs are very rare, and their prevalence has been reported at 5-6% in the clinic. Adrenal CYSTs comprise 1% to 22% of incidentally discovered adrenal masses. Given the rarity of adrenal CYSTs and their asymptomatic occurrence in some patients, in this study, a patient with an adrenal CYST referred by weight gain is introduced. Patient introduction: A 49-year-old female patient referred with a complaint of abdominal swelling and gradual weight gain since a year ago. In ultrasound examinations, she was found to have an abdominal mass of unknown origin. In the computed tomography (CT) scan with and without contrast of the patient, the image of a 32 × 28 × 40 cm CYST with a thin wall and transparent contents in the pelvic area showed the appearance of an adrenal pseudoCYST, which has caused a pressure effect on the right kidney and its displacement to the left retroperitoneum. According to laboratory tests and repeated visits, the patient underwent surgery with a possible diagnosis of adrenal mass. The patient's complaints were resolved after the surgery. Conclusion: In general, the present study was related to adrenal CYSTs with uncommon manifestations. The diagnosis was made with the help of a CT scan before the surgery, and the patient's surgery was performed without any major or minor complications. However, if the centers are equipped with the appropriate skills and facilities, laparoscopic management of these patients is suggested.

Abstract:

In this study a SIMPLE and rapid method was used for the preparation of hydatid CYST B-group antigens. In this regard, after the elimination of albumin and globulin from the CYST hydatid fluid, the antigen fluid was derived including B-group and 5-group antigens. The 5-group antigen was removed from the fluid by boiling. To determine the purity degree and molecular weight of the remained antigens, the Native-PAGE and SDS-P AGE methods were used. For evaluating the antigenicity of the fluid Dot-ELISA was employed. The result showed that , B-gruop antigen contains 8-12 , 16 and 20-24 KDa proteins. These antigens can be used for hydatid CYST diagnosis with high specificity specially by Dot-ELISA technique.
